#5 - The Process and Plan of Care for a Comprehensive Pulmonary Rehabilitation Program

I. Four domains of care required for delivery of a comprehensive pulmonary rehabilitation program.

II. The rehabilitation process.

III. Individualized treatment plan (ITP) including the four domains of care and the rehabilitation process.

IV. Proper billing for the pulmonary rehabilitation services provided.

V. Documentation for the services provided.

1. Explain the four domains of care required for a comprehensive pulmonary rehabilitation program.
2. Describe the rehabilitation process of assessment, goal setting, interventions and reassessment.

#6 - Pulmonary Rehabilitation Case Examples and Individualized Treatment Plans

I. Utilize assessment findings from the case example to identify goals and interventions.

II. Discuss the Medicare required 30 day reassessment.

III. Identify the specific billing code(s) and the documentation for the services provided.

IV. The discharge process.
